Consent and Waiver

Vaccinations: Diamonds and Dutch Pet Bath & Spa require all puppies aged 10 weeks and older to have their first two rounds of vaccinations before receiving any services. For pets over 4 months of age, a rabies vaccination is also required. Please provide copies of these documents before any services can be performed.

Health | Medical Problems or Senior Dogs: Grooming procedures can sometimes be stressful, especially for senior pets or those with health issues. As a result, grooming may reveal hidden medical problems or aggravate existing ones, either during or after the session. To prioritize the safety and comfort of senior pets and those with health concerns, we will focus on cleanliness rather than achieving a “perfect groom.” Pet owners must inform us of any medical, physical, or emotional issues, allergies, sensitivities, or pre-existing conditions. This includes, but is not limited to, prior surgeries, hip or joint issues, warts, moles, ear infections, and skin problems. Pet owners should also update us about any new conditions as they arise. We cannot be held responsible for any accidents or injuries that occur during the grooming of elderly or health-compromised pets. By signing below, the pet owner confirms that they have permission for us to groom their senior pet(s) and have discussed all relevant health issues, both past and present.

Grooming | Bathing: Prices provided over the phone prior to your appointment are ESTIMATES and NOT guaranteed. When you drop off your pet(s), a staff member will provide final pricing after a grooming consultation. Grooming prices may change at the groomer's discretion based on factors such as the desired style, coat condition, and your pet's temperament. Our groomers will evaluate your pet(s) and discuss the desired trimming options with you. Please note that certain trims and cuts are specific to certain breeds, and our groomers will do their best to achieve the look you want. If your request cannot be fulfilled, the groomer will suggest alternative cuts or trims. If there are any changes to the price after the consultation, we will call you to discuss your pet’s coat and skin maintenance.

Fleas | Ticks: If fleas or ticks are found on your pet(s), it is our policy to administer a flea and tick bath at an additional cost of $15.00 per bottle. We will notify you as soon as possible. This fee covers the flea/tick shampoo, as well as the time and extra cleanup needed to ensure no fleas or ticks remain in our spa. While the shampoo effectively kills fleas and ticks on your pet(s), it will not prevent re-infestation if fleas or ticks are present in your home or pet's environment. We always recommend that pet owners research and implement a flea and tick preventative as part of their pet’s health care routine.

Matted Dog Policy: Pets with matted coats require extra time and attention. If the matting is minimal and your pet responds well, we can brush or trim their hair for an additional fee of $15 to $30. However, if we believe that removing the mats will take too long or if your pet seems to be in pain, we will stop the service and recommend scheduling a visit with your vet. We believe grooming should be a positive experience, and we will not subject your pet to unnecessary discomfort from de-matting. Matted fur increases the risk of nicks, scratches, and cuts during grooming. Additionally, the skin underneath may appear red, itchy, and irritated due to lack of oxygen. We cannot be held responsible for any injuries sustained while grooming a matted pet, including any aftereffects. Our groomers are happy to demonstrate effective brushing techniques to help prevent matting. Regular brushing at home with a brush and comb, along with a grooming schedule every 4 to 6 weeks, can significantly reduce matting.

Dangerous/Aggressive Animals or Behavior Issues: Pet owners must inform the groomer if their pet has a history of biting, aggression, unpredictability, or any other behavioral issues. Appointments outside of normal business hours are available for an additional charge if needed. If the pet owner fails to notify us of any potential dangers or behavioral concerns, they may be liable for all medical fees, lost work compensation, and property damage. All bites will be reported to local authorities as required by law. We will make every effort to groom uncooperative pets; however, we may not be able to complete grooming on pets that pose a threat to themselves, other pets, or our groomers. We reserve the right to muzzle any dog for the safety of everyone involved. Muzzling does not harm your pet and can help calm a stressed animal, allowing the grooming process to continue. Diamonds and Dutch Pet Bath & Spa reserves the right to refuse, pause, or cancel grooming services at any time. For extremely aggressive or unruly pets where grooming must be paused, the customer will be charged HALF of the grooming fee plus a $20 STOP SERVICE fee. Pet owners may return within the same week to complete the grooming, but they MUST call to confirm the appointment within 48 hours of the paused grooming. Upon completion, the pet owner will pay the remaining balance. If the grooming cannot be completed, the owner must return for immediate pickup, and the pet will be placed on the “Do Not Book” list.

Muzzle: Muzzling does not harm your pet and protects both the pet and the groomer. In some cases, muzzling may even calm a stressed animal, allowing the grooming process to continue. If your pet still acts in a way that is dangerous, we have the right to stop grooming services at any time and a service fee will be collected. We do not muzzle unless your pet gives us a reason to.

Late Fees/Reschedule & Cancellations/No show policy: Unfortunately due to high grooming and bathing demands, new customers & existing customers not in good standing will be charged 45% of their appointment fee as a deposit to secure their appointment. If a pet owner needs to reschedule or cancel their appointment, they should contact Diamonds and Dutch 48 hours prior to said appointment. If notified 72 hours prior, deposit fees can be transferred to the rescheduled appointment time and/or date. If the customer does not inform Diamonds and Dutch and does not have a valid reason for not calling, the customer may forfeit their deposit. A late fee of $ 15 will accrue 10 minutes after scheduled appointment and an automatic $25 fee will be added after 20 minutes. Accepting late arrivals is at the bathers’/groomers’ discretion. If they cannot accept your pet, then you will need to reschedule for a different date. A customer can be considered a no show if they have not contacted the salon or arrived for their appointment 20 minutes after the time it was set for and will forfeit their deposit. A “No Call/No-Show” fee of $25 can/will be added to the customer’s account and MUST be paid to book future appointments. For Pick Up: Customers have 30 mins after being verbally notified of pick up. Arrival after 30 mins will accrue $15 late fees for every 30 mins. If customers obtain more than (3) cancellations or (3) no shows within one year, the customer will be required to pay the full-service amount prior to appointment. Customer can be suspended from grooming or bathing services if customer exceeds limits. Customers who have not rescheduled an appointment within the past 3 months may be considered a new customer and will receive updated pricing.

Depending on schedule availability, a $15 (per pet) same-day fee may apply for squeeze-in appointments. Please note that Fridays-Sundays are peak times at Diamonds and Dutch Pet Bath & Spa, making these days particularly popular for services. To ensure your pet secures a spot for the weekend, an additional charge may be added to the service price, depending on demand.

Satisfaction: Your satisfaction is important to us. If you are unhappy for any reason and would like adjustments made, we will be happy to accommodate you when you pick up your pet after their appointment. However, once you take your pet home, we will only accept requests for adjustments within 48 hours of the appointment. Any adjustments requested after this time will incur a grooming fee. We may also take before-and-after photos of your pet for our website, advertising, and educational purposes. By signing this consent form, you indicate your understanding and agreement to our terms of service, including the release of Diamonds and Dutch Pet Bath and Spa, its owners, employees, and affiliates from all liabilities, expenses, damages, and costs (including attorney fees) resulting from any service provided or any injury (including death) to your pet while in our care or afterward. We ask that clients resign this consent form every six months to ensure they are updated with our shop policies.
